Court forms

There are two types of court forms. Both are publicly available, but they serve different purposes:

  • Commonly used forms — the core forms most often needed in family and civil cases. They are grouped by topic below for easier access.
  • Court rules formsthe full index of forms set out in the court rules. These are detailed and procedural, and are mainly used in legal practice.

    Gender recognition – court forms

    These forms are used for Circuit Court applications concerning gender recognition. They are mainly required where the applicant is under 18, or where you are appealing a Minister’s decision. Always check you are using the correct version before filing.
