Court forms

There are two types of court forms. Both are publicly available, but they serve different purposes:

  • Forms for the public — everyday forms for people representing themselves or making common applications. They are grouped by topic for easier access.
  • Forms for legal professionals — the full index of forms set out in the court rules. These are technical resources mainly used by solicitors, barristers, and court staff, and require knowledge of court procedure.

    Gender recognition – court forms

    These forms are used for Circuit Court applications concerning gender recognition. They are mainly required where the applicant is under 18, or where you are appealing a Minister’s decision. Always check you are using the correct version before filing.
